Please use this form to request campus services and/or departments needed for your Event. Form should be filled out entirely with as much detail as possible. Each service or department will reach out if more information is needed. If you have questions please email campusoperations@frontier.edu.
How to create an event using the FNU Event form.
BT Portal > Department> Campus Operations> Forms> Event Form
Information needed will be as follows
Email: ( please use FNU email address)
Event Name: Insert the name of event
Description: A brief description of the event.
Department/Service (for billing):
Est number of people: How many people to you except to attend
Event organizer: Who will be the point of contact for the event.
Start Date: When will the event start.
Start Time: Actual start time
End Date: When will the event end.
End time: Actual end time
Summary: Brief summary of any needs or wants for the event
Select your Room Setup
Please select one setup if you would like the conference room rearranged.

Dining Services/Catering
You will be able to select from the catering menu and inform dining staff or any allergies. Please note set up time for this service may be different than event set up time
Operations Services (housekeeping or housing services)
If you will be needing Housing or cleaning services after your event. Are you needing anything ordered for this event? If you are needing items ordered that are not food related please fill out a materials request form found on the BT portal.
Facilities Services
This service helps with space set up. Will you need tables or chairs moved or set up in a different formation?
IT support
WIll you need IT to set up or help support the event? Presenting or needing IT equipment?
Other services
If you are needing a service not listed or provided please let us know by selecting this option and be as detailed as possible. Other services could include outsourced catering, rental company for tables and chairs, or involving other departments/services on campus not listed. It will be the responsibility of the event creator to inform those departments not listed on this form about the event.
Attachments: Attach any files or images that may help in the set up or execution of this event.
I.e. Guest list, contact information, pictures of set up, ect
After the form is submitted, the Services selected will be notified by email of your needs. You will also receive a confirmation email from operations that your event was created and received. Your event will be added to the FNU Google calendar. Please allow three business days for all Services to respond. If there are any changes to the number of attendees, you will need to notify Campus Operations and the Food Services Manager five business days in advance. The cost of dining services per attendee you have indicated will be billed to your budget.
