As the SNF-ist, you’ll serve as the clinical leader in a Skilled Nursing Facility, overseeing care for both short- and long-term residents. You’ll provide assessments, prescribe treatments, support advanced care planning, and collaborate with physicians and nursing staff to ensure high-quality patient care.

What We’re Looking For

• Master’s degree and active APN license (DEA # required)
• Skilled Nursing experience a plus, but not required
• Strong communication, teamwork, and leadership skills
